Welcome to Bridges to Wellness!
Bridges to Wellness/Puentes a la Salud is a 10 year, resident driven initiative which began in 2008 with funding from the Greater Rochester Health Foundation through the Neighborhood Health Status Improvement Program. The initiative seeks to improve the personal and community health of approximately 1,500 individuals who reside in the segment of Rochester bound by Avenue D and Nash Street to the north and south, as well as Portland and Hudson to the east and west, respectively. Focus groups with community residents narrowed the scope of their efforts to concentrate on four areas of community health:
- Public and Personal Safety
- Personal Development
- Community Pride and Identity
- Mobility and Transportation
